Prying eyes: Protecting patient records

Electronic access to patient data has made it easier to look up information -- sometimes too easy.

You've probably heard stories about employees or others tapping patient information systems for identity theft. But the more frequent problem is snooping -- curious staff or others with system access who look at information they're not authorized to see.

It sounds innocent, but HIPAA and an increasing number of state laws that cover disclosure of information breaches don't make distinctions based on intent. An information breach is an information breach, which means physician practices not only have to find ways to keep gawkers away but also must be ready to carry out consequences -- or face them -- if a breach occurs.

A case in point: New Hampshire Orthopedics in Manchester, N.H., discovered that someone who wasn't supposed to look at a patient's information had done so -- and was telling others about what was in the file.

An employee had tapped into the electronic medical record system of Elliot Health System, which the practice and employees were authorized to use. The patient whose files were breached was an employee at the orthopedic practice. When workers started to gossip about what they had found, a practice manager contacted Elliot, which used a system audit to trace the breach back to two employees.

New Hampshire Orthopedics fired the employees, but Elliot was in the hot seat, as state law required the hospital to file a disclosure and contact the patient. The practice's name was included in the publicly available report.

Best type of Creatine?

Question: I’m out of creatine and I want to try something new any suggestions? Just looking for personal favorites here. I was using SizeOn and SuperPump. They are both ridiculously expensive i’m looking for something thats somewhat cheap.

Answer 1: The kind that comes in Steak.

Answer 2: Super pump 250 is not creatine. Its an NO booster. Try Now Foods creatine. Good brand, cheap. Or Optimum Nutrition. Also stay away from the NO boosters, they do pretty much nothing.

Answer 3: I liked Cell Tech when I was taking it.

Answer 4: I take any Creatine Ethyl Ester.

Answer 5: Plain old Creatine Monohydrate. Anything “added in” is a waste of your money. You don’t need vitamins in your creatine.. you don’t need special this or special that… and you don’t need a “loading phase” your first week or so. Plain old Creatine Monohydrate.

Answer 6: No explode has creatine and NO which all help you. I was on it over the summer and my bench went up 90 pounds in a lil more then a month. But idk what you want to do. Get toned or gain weight because it makes a huge difference. What do you want to do? btw im on super pump and size on right now and nitrotech hardcore and i love all of them….Nitrotech hardcore is really good for gaining weight

Answer 7: Cell Mass. Yes, it’s true, you can buy the cheap, plain creatine, add juice and you’re good. But, if you have the $, Cell Mass is probably the best out right now. It works. Take it twice a day - on an empty stomach…once right after training.

Answer 8: Stick to plain old creatine monohydrate. Everything else is overpriced and a waste of money. Can’t really stress enough how pointless the other products are, they do they same thing as regular monohydrate (when taken right) for literally 3 to 4 to 5 times the price.
